The birth certificate is the easiest way to prove guardianship however if you do not have access to your birth certificate or have a guardian not involved in your birth, these are your options...
IF YOU AND YOUR GUARDIANS LAST NAMES MATCH: It's okay if you do not have access to your birth certificate in this case. We will only need photo ID's. So, guardians drivers license and your school (or other photo) ID.
IF YOUR LAST NAMES DO NOT MATCH:
For step parents: Please bring marriage certificate to childs parent.
For guardianship from other reasons: Please bring court papers deeming you legal guardian of child.
MAIDEN NAME DISCREPENCY: If mothers maiden name is on birth certifcate, child has birth fathers last name, but mother has re-married another man and has different last name than child and on birth certificate.
YES, THIS IS ACCEPTABLE: In this special scenario, mother will be presenting ID and it's assumed very difficult to find someone to sign consent for you with the same first and middle name as your mother, that is not your mother.
WHAT IF I DO NOT HAVE A SCHOOL ID?
WE ALSO ACCEPT: A Passport, a safe child card (sheriff ID), a military ID, a yearbook itself (with a picture of you in it and your name printed under the picture), a sports picture that has your name printed on it (not written on it).
